Services provided
Social work services are available to assist patients and their families during hospital admissions or at outpatient visits. Continuity of care from pre-admission to post-discharge is provided whenever possible.
Social work staff work in specialised areas throughout the hospital providing a range of expertise in areas such as: - Emotional wellbeing including individual, couple and family counselling - Discharge planning - Financial and community support services - Consultation and co-ordination where infants are considered "at risk" of neglect or abuse
Specialised services
Oncology Psychological and practical support is provided for all oncology patients. Linkage to local services, support groups and hospital based programs.
Special Care Nurseries Psychological and practical support is provided for families of all babies in Special Care Nurseries. The Social Work Department coordinates a peer support group for nursery parents.
Transitions Clinic Ongoing case management for maternity patients with complex needs (teenage pregnancies, substance abuse issues, Aboriginal women). See separate listing in this directory.
Obstetrics and Gynaecology Women attending these Units are seen by referral to the Social Work Dept, or self-referral.
Aboriginal Women and Family Support Unit The Unit comprises a full- time Aboriginal Hospital Liaison Officer and a part-time Community Health Worker. They are linked to the Social Work Department, and provide ongoing care to all Aboriginal women attending the hospital as inpatients or outpatients.
